Referrer Information
Individuals wishing to apply for a grant from Berkshire Nurses and Relief in Sickness Trust require a referrer to submit their claim to us.
We cannot accept applications direct from members of the public. As the referrer, you should be known to the applicant in a professional capacity and be able to help them with completing the form
Here is a quick checklist and some information for your use: -
- All referrals must be made by a member of a statutory or voluntary agency, health professional or similar.
- The referrer must not be a potential beneficiary of the grant (e.g. a counsellor may not support an application for their own service)
- The referrer should not be related to or know the applicant in a personal capacity.
- Ensure all parts of the forms are completed – incomplete forms will be returned. The applicant MUST sign the application form and consent forms themselves.
- Supporting medical evidence will be needed – this can be a letter from a medical professional, details of a PIP award or details of prescribed medications for example. Please ensure this is emailed with the application form.
- Please provide a supporting letter giving as much information as you can regarding the reason for the grant application and how you think this will benefit the applicant
